It is just a combination of those two. The laws of refraction and reflection apply.
For simplicity the two are usually not drawn in one image, but they should be.
If you are not aware of this then crazy things like "total refraction" are taught where it shouldn't be. When there is such a thing as total refraction, it doesn't apply in this simplified case.
The key here is the "total" in total internal reflection. Which seperates it from reflection.
I uploaded a few homebrew animations to show what happens.
First one is refraction plus refraction (light coming from and and traveling to/in water),
second one is total internal reflection (or reflection alone).
