Are those inclusions are natural? RI 1.76 , SG 4 .

Hard to determine from the picture if the roundish inclusions are bubbles or crystals.
The long needle-like things : are these scratches on top of the stone or are they inside ?
If inside, I would say natural stone (corundum).

AS Isi pointed out, the needle inclusions appear to intersect at 60º which is diagnostic for corundum.

With the caution that all we have seen is a single picture on the web, I would say natural, unheated, Ruby.

In most parts of the World, that tone would definitely qualify as Ruby.
